ABSTRACT

Objective To systematically review the epidemiological studies on primary biliary cholangitis (PBC), and to investigate the prevalence rate of PBC in the Chinese general population and its influencing factors. Methods PubMed, Embase, The Cochrane Library, CNKI, and Wanfang Data were searched for articles on the epidemiology of PBC in China published up to 31th March 2022. Two researchers independently performed screening and data extraction, and then related analyses were performed. Results A total of 9 articles were included. The positive rate of AMA was 1 049.05/100 000 (ranging fr om 159.65/100 000 to 2287.40/100 000), and the prevalence rate of PBC was 123.68/100 000 (ranging from 42.70/100 000 to 276.59/100 000). The positive rate of AMA was 636.51/100 000 (ranging from 52.55/100 000 to 1 164.33/100 000) in men and 1 265.47/100 000 (ranging from 225.23/100 000 to 1 704.93/100 000) in women, with a male/female ratio of 1∶1.99 for the prevalence rate of AMA. The prevalence rate of PBC was 40.81/100 000 (ranging from 23.54/100 000 to 75.10/100 000) in men and 148.71/100 000 (ranging from 77.36/100 000 to 214.91/100 000) in women, with a male/female ratio of 1∶3.64 for the prevalence rate of PBC. Conclusion Different studies show great differences in the positive rate of AMA and the prevalence rate of PBC in the Chinese general population, which is mainly affected by sex, age, and region. The positive rate of AMA and the prevalence rate of PBC increase with age, and the patients aged ≥50 years have a significantly higher positive rate of AMA than those aged < 50 years. The positive rate of AMA is significantly higher than the prevalence rate of PBC. There are significantly more women than men in the AMA-positive population and the PBC patients, and the influence of sex on AMA is lower than that on PBC.

ABSTRACT

BACKGROUND: Intra-articular injection of sodium hyaluronate can safely and effectively treat knee osteoarthritis and obviously improves the pain and function of knee joint. The efficacy of extracorporeal shock wave therapy for early-and middle-term knee osteoarthritis still remains controversial. OBJECTIVE: To compare the effects of extracorporeal shock wave therapy and intra-articular injection of sodium hyaluronate for early-and middle-term knee osteoarthritis. METHODS: According to the inclusion criteria, a total of 77 patients with knee osteoarthritis were enrolled, and randomly divided into extracorporeal shock wave therapy group and sodium hyaluronate therapy group. The extracorporeal shock wave therapy group underwent 5 sessions of 2 000 shockwave pulses performed on the affected knee with the dosage adjusted to 0.2 mJ/mm2 energy. The sodium hyaluronate group was administered intra-articularly sodium hyaluronate once per week for 5 weeks. The results were assessed with the Visual Analogue Scale, Lequesne index and Western Ontario and McMaster Universities Osteoarthritis Index. RESULTS AND CONCLUSION:① After 5 weeks of treatment, the Visual Analogue Scale, Western Ontario and McMaster Universities Osteoarthritis Index and Lequesne index in the two groups were significantly decreased compared with the baseline (P < 0.05). All above indexes in the extracorporeal shock wave therapy group were significantly lower than those in the sodium hyaluronate therapy group (P < 0.05).② These results indicate that the extracorporeal shock wave therapy and sodium hyaluronate therapy are effective for early-and middle-term knee osteoarthritis, and the former is more significant.

ABSTRACT

A set of device for the measurement of the pressure difference between the anterior and the posterior chambers (PDAP) was designed to investigate the temporal varying rules of PDAP in the anterior segment of rabbit eyes. A platform was established for the measurement of PDPA according to the mechanism of joint implement. Rabbit models with high intraocular pressure (IOP) were constructed by means of injecting Carbomer into anterior chamber to increase IOP. The 24 hours continuous measurements of PDAP were performed for normal rabbit eye and eye with high IOP. The developed device could sensitively response to the small pressure difference in eye. The pressure difference in the normal rabbit eye varied with time, and the variation range during a whole day was 5.84-96.84 Pa which reflected the existence of physiological rule. For the rabbit eye with high IOP, pressure in anterior chamber was higher than that in posterior chamber which was in consistence with the theory of self-adaptation adjustment. The present study indicates that the approaches and device designed in this paper can well implement the measurement of PDAP as well as the temporal varying rules of PDAP in the anterior segment during a whole day.

ABSTRACT

BACKGROUND: Glaucoma is a kind of eye disease that can cause irreversible blindness which is characterized by visual field loss. Clinical research shows that the optic nerve head has changed before the visual field loss. The morphological changes of the optic nerve head have become the key to determine the early diagnosis of glaucoma and disease development. So it has important significance for us to study the morphological changes of the tissues of optic nerve head under the high intraocular pressure. OBJECTIVE: To establish three-dimensional finite element model of optic nerve head which includes choroid, retina and lamina cribrosa, and analyze the thickness of the optic nerve head under the acute high intraocular pressure. METHODS: (1) We chose healthy cats without refractive media and other eye diseases. The tomographic sequence images of the optic nerve head of a cat were obtained with the intraocular pressure of the normal value by using enhanced depth imaging spectral-domain optical coherence tomography. (2) The three-dimensional model of the retina, choroid and lamina cribrosa was obtained. Three-dimensional model of optic nerve head was established by assembling the model of the retina, the choroid and the lamina cribrosa. The thickness of the retina, choroid and lamina cribrosa was analyzed under different intraocular pressures by using the method of finite element analysis. (3) Animal model of acute ocular hypertension was established by methods of anterior chamber perfusion. The tomographic sequence images of the optic nerve head of a cat were obtained with different intraocular pressures by using enhanced depth imaging spectral-domain optical coherence tomography. Then, we measured the thickness changes of the choroid, retina and lamina cribrosa under different intraocular pressures, and compared with the results of finite element analysis. RESULTS AND CONCLUSION: The thickness of the choroid, retina and lamina cribrosa trended to be thin, and the cup dish ratio of optic nerve head increased gradual y with the increased intraocular pressures. Regarding to the thickness changes of the choroid, retina and lamina cribrosa, the trend of experimental measurement results was consistent with finite element calculation results. Hence, it is feasible to analyze morphological changes of every fundus tissue under high intraocular pressure using the tomographic sequence images obtained by optical coherence tomography. We can predict the morphological changes of the optic nerve head tissue by finite element analysis, which has certain guiding significance in determining the progression of glaucoma.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the efficacy on obesity of spleen deficiency and damp blockage pattern and hyperlipemia treated with warm needling therapy and auricular acupuncture and the effect mechanism of the combined treatment.</p><p><b>METHODS</b>One hundred and ten patients of obesity of spleen deficiency and damp blockage pattern and hyperlipemia were randomized into an observation group and a control group, 55 cases in each one. Additionally, a healthy group (52 cases) was set up as the control. In the control group, the warm needling therapy was applied to Taibai (SP 3), Chongyang (ST 42), Yinlingquan (SP 9), Zusanli (ST 36), etc., once every two days. In the observation group, on the basic treatment as the control group, the auricular acupuncture was applied to Pi (CO₁₃), Wei (CO₄), Fei (CO₁₄), Shen (CO₁₀), etc., once every 2 to 3 days. The efficacy was evaluated after 3-month, treatment in the two groups. The observation was conducted on the obesity outcomes [body mass, obesity degree (A), body mass index (BMI), body fat percentage (F%)], blood-lipoids indicators [such as serum total cholesterol (TC), triacylglycerol (TG), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C)], fat-islet endocrine axis outcomes [such as fasting plasma glucose (FPG) , fasting leptin (FLP), fasting insulin (FINS), insulin sensitivity index (ISI), insulin resistance index (Homa-IR) and insulin β cell function index (Homa-β)], as well as autonomic nerve function index (Y) before and after treatment in the patients of the two groups. The efficacy was assessed in the two groups.</p><p><b>RESULTS</b>The total effective rate was 96.4% (53/55) in the observation group, better than 87.3% (48/55, P < 0.01) in the control group. For the improvements of the obesity indices, the differences were not significant between the two groups (all P > 0.05). Before treatment, the levels of TC, TG, LDL-C, FLP, FPG, FINS and Homa-IR in the two groups were all significantly higher than those in the healthy group (all P < 0.01), and the levels of HDL, ISI, Homa-β and Y were significantly lower than those in the healthy group (all P < 0.01). After treatment, except Homa-β, the other indices were all improved significantly (all P < 0.01). The results in the observation group were better than those in the control group (all P < 0.01).</p><p><b>CONCLUSION</b>The patients of obesity of spleen deficiency and damp blockage pattern and hyperlipemia have the disturbances of lipid metabolism, "fat-islet endocrinal axis" function and automatic nerve function. The combined treatment of warm needling therapy and auricular acupuncture or simple warm needling therapy present the effects of weight reducing and lipid reducing. The effect of the combined treatment is better than simple warm needling therapy. The efficacy mechanism is probably relevant to the positive regulation of blood glucose, lipid metabolism, fat-islet endocrinal axis and automatic nerve function.</p>

ABSTRACT

This study was aimed to observe effect of combined therapy of acupuncture, tapping and auricular needling on fatty-insulin endocrine axis in obesity complicated with hyperlipidaemia patients withyin-deficiency and blood-stasis pattern. A total of 108 patients with obesity complicated with hyperlipidaemia ofyin-deficiency and blood-stasis pattern were randomly divided into the treatment group of combined therapy of acupuncture, tapping and auricular needling, and the acupuncture and tapping group, with 54 cases in each group. The comparison was made to 52 patients in the normal group. Observations were made on changes of fatty-insulin endocrine axis index, lipid index, obesity index, vegetative nerve function index (Y value) and clinical weight-reducing and lipid-reducing effect. The results showed that the pretreatment obesity index, fasting insulin (FINS), fasting plasma glucose (FPG), insulin resistance index (Homa-IR), fasting leptin (FLP), triglyceride (TG), total cholesterol (TC), low density lipoprotein (LDL-C) and Y value of patients in both groups were significantly higher than those of the normal group (P < 0.01). However, the high density lipoprotein (HDL-C) and insulin sensitivity index (ISI) were significantly lower than those of the normal group (P < 0.01). After treatment, both groups received satisfied effects on weight-reducing and lipid-reducing. The fatty-insulin endocrine axis indexes (FINS, FPG, Homa-IR and FLP) were significantly reduced (P < 0.05); and ISI was significantly increased (P < 0.05). The combined therapy of acupuncture, tapping and auricular needling had better regulation effects on the fatty-insulin endocrine axis level, lipid level and vegetative nerve function level compared with the acupuncture and tapping group (P < 0.05). It was concluded that patients with obesity complicated with hyperlipidaemia ofyin-deficiency and blood-stasis pattern had dysfunction of the fatty-insulin endocrine axis. The combined therapy of acupuncture, tapping and auricular needling, or the therapy of acupuncture and tapping had good regulation effect. However, the combined therapy of acupuncture, tapping and auricular needling had more significant therapeutic effect.

ABSTRACT

OBJECTIVE@#To provide a clinical reference by comparing the conventional 226 Hz tympanometry with 1000 Hz tympanometry in two groups of young children with otitis media effusion evidenced by CT scan.@*METHOD@#One hundred and seventy-seven young children (226 ears), from 1 to 60 months, with otitis media effusion were involved in this study. They were divided into six groups by age: 0-6 months group, 6-12 months group, 12-18 months group, 18-24 months group, 24-36 months group, 36-60 months group. They were tested with tympanometry of 2 probe-tones of 226 and 1 000 Hz. Type A tympanogram was defined as a normal middle ear function in 226 Hz and single-or double-peak in 1 000 Hz tympanometry. One hundred and fifty-eight normal young children (266 ears) were selected as control group. The results were analysed with Chi square test. Receiver operator characteristic (ROC) analysis was performed to evaluate the two methods.@*RESULT@#In the young children with otitis media effusion, the detection rate of 226 Hz tympanogram in six groups was 21.1%, 35.2%, 46.9%, 42%, 62.5% and 68% respectively, while 94.7%, 98.1%, 96.9%, 91.2%, 95.8% and 88% respectively in 1 000 Hz tympanogram. In the young children with normal middle ear function, the detection rate of 226 Hz tympanogram in six groups was 95.1%, 88.6%, 85.1%, 93.3%, 88.5% and 93.5%, while 87.8%, 94.3%, 89.4%, 95.6%, 94.2% and 97.8% respectively in 1 000 Hz tympanogram. The detection rate was significantly different between 226 and 1 000 Hz tympanogram in the young children under 36-month old.@*CONCLUSION@#A single-or double-peak 1000 Hz tympanometric patterns as normal criteria was a simple way to evaluate young children s' middle ear function. 1 000 Hz tympanometry should be given priority to the children within 36-month old in detection middle ear function, the 226 and 1 000 Hz tympanometry should be done at the same time within 36-60 months old.

ABSTRACT

This study was aimed to observe the effect of electro-acupuncture (EA) treatment on Zhong-Xiao (ZX, T2MD) with obesity. A total of 203 patients with ZX accompanied with obesity accepted EA treatment. Before and after 3-month treatment, observations were made on patients' fasting blood glucose (FBS), fasting insulin (FINS), in-sulin sensitivity index (ISI), insulin resistance index (Homa-IR), insulin secretion index (Homa-β), degree of obesity (A), body mass (Kg), body mass index (BMI) and body fat percentage (F%) changes. The results showed that com-pared with pretreatment, the posttreatment FINS, Homa-IR, FBS had a significant decrease among 203 patients (P<0.01); the ISI and Homa-β had a significant increase (P < 0.01). There was a significant statistical difference on islet β-cell function before and after treatment. The A, body mass, BMI, and F% were significantly decreased (P<0.01). It was concluded that EA can significantly correct ZX with obesity patients' glucose consumption, protect the islet β-cell function, and reduce the body mass of patients.

ABSTRACT

This study was aimed to observe the clinical effect of acupuncture together with warm acupuncture on fe-male type 2 diabetes mellitus (T2DM) patients of dual deficiency of yin and yang syndrome. A total of 92 female T2DM patients of dual deficiency of yin and yang syndrome were divided into two groups, which were priority of yin deficiency in the type of dual deficiency of yin and yang (group 1) and priority of yang deficiency in the type of dual deficiency of yin and yang (group 2). There were 46 cases in each group. The treatment of acupuncture together with warm acupuncture was given for three month. The observation was made on the fasting plasma glucose (FPG), fasting leptin (FLP), fasting insulin (FINS), insulin sensitivity index (ISI), insulin resistance index (Homa-IR), insulin secre-tion index (Homa-β), total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), high den-sity lipoprotein cholesterol (HDL-C), body mass, body mass index (BMI) and body fat percentage (F%) before and af-ter treatment. The results showed that the total effective rate of group 1 was 69.57%, while the total effective rate of group 2 was 84.78%. There was no significant statistical difference between two groups. Compared with pretreatment, there were significant decrease on FLP, FPG, FINS, Homa-IR, TC, TG, LDL-C, body mass, BMI, and F% of both groups after treatment (P < 0.01). There was obvious increase on ISI, Homa-β, and HDL-C (P < 0.01). After treat-ment, the FPG, Homa-β and BMI of group 2 were better than group 1 (P< 0.05). After treatment, the level of LDC-C and HDC-L of both groups reached normal level. And the FPG of group 2 also reached normal level. It was con-cluded that acupuncture together with warm acupuncture can significantly adjust the level of fat-pancreatic islet shaft of T2DM patients of dual deficiency of yin and yang syndrome, reduce the leptin resistance and insulin resistance, enhance the sensibility of insulin and the function of pancreatic β cell, and correct the endocrine disorder, gly-cometabolism disorder and lipometabolism disorder. Acupuncture together with warm acupuncture did much better in lowering blood sugar and improving the function of pancreatic β cells of T2DM patients whose priority of yang defi-ciency in the type of dual deficiency of yin and yang than those whose priority of yin deficiency in the type of dual deficiency of yin and yang.
